This form collects personal information via unsecured email. A.D.A.M., Inc. will not without your permission, give out your name, company, phone number, residence address, or e-mail address to any third parties, for any reason, at any time, ever. A.D.A.M. editorial associates will review the request, qualify the request and enter your supplied information into a secure database, accessible only to select A.D.A.M. employees. This information is stored in the A.D.A.M. database indefinitely, or until you submit a request for the information to be removed. You may request that your information be deleted from the A.D.A.M. Sales database at any time by clicking the "Remove My Contact Information" link supplied above.

Any personable identifiable information (PII) provided to us through the use of this form will be used for the sole purpose of identification in the event we need to contact you for clarification. We will not respond to you unless specifically requested to do so, or in the event we have questions regarding your comment. If a response is requested, we will make every attempt to contact you within three (3) business days.
